Please complete and return all the required forms along with the annual registration fee to The Willow CDC Office.
Enrollment is open through out the year for new students (if space is available).
Other required forms:
- Child's Vaccination Records (required for every student)
- Preschool Health Statement (required for every student)
- Hearing & Vision Screening Test (for Pre-Kindergarten students only)
